CROWLEY'S RIDGE SILVER LABS 31752 Co. Rd. 309 Advance, MO 63730 573-421-8777 (Date) Greetings, Congratulations on your new family member! Crowley's Ridge Silver Labs thank you for allowing us to provide you and your family with one of the finest, most enjoyable and loving additions to your home. Should at any time, you would have questions or concerns, we will do everything possible to assist you and help complete satisfaction. GENERAL INFORMATION Maintaining good health is essential to top performance for your pet. Avoid table scraps and over eating habits. All our dogs and puppies are fed TLC brand dog food. Medical treatment and protection is important against heart worms, and flea and tick control. Discuss what is best for your pup with your veterinarian. Your puppy was given its first set of vaccinations before eight weeks of age. Always consult with your vet concerning the updates on vaccinations for protection. The veterinarian can recommend benefits that maintain your puppy's health, and we have always been supportive of NuVet Supplemental vitamins, but not as a requirement. All dogs at Crowley's Ridge have used NuVet supplements since we purchased them at 8 weeks of age and it was observed that our dogs have less fur shed than other labs. A Labrador Retriever requires space and exercise daily as well as interaction with your family. This breed is naturally inquisitive and intelligent. You may need to help protect your pup and your belongings from chewing valuables or dangerous items (electrical cords) as they learn what is safe. Health Warranty Crowley Ridge guarantees your pup to be free from all hereditary diseases determined by the emBark testing, illnesses and/or disabling conditions for a period of 12 months from date of whelp. Your pup is free from disabling hip and/or elbow dysplasia for a period of 25 months from date of whelp. “Disabling” means that a pup requires immediate surgery or surgery within 25 months of warranty; or is significantly lame and unable to engage in normal activities without substantial loss of function – ability to run, climb and/or jump. *To preserve this warranty you must prevent your pup from becoming obese, exposed to injury or abuse; and if a female, from being bred during the first 25 months of life. *Should you breed your female pup during this 25 month period, the hip/elbow guarantee is void. *CRSL follows and implements the OFA (Orthopedic Foundation of Animals) guidelines for evaluation and grading of hip/elbow dysplasia. *Hip/elbow dysplasia is not a fully understood condition. It is believed to be caused by a combination of hereditary genes, environmental (abuse or injury) and/or nutritional (food and supplement) factors. It may not appear until after 12 months of age and OFA only permits evaluation after 24 months of age. Only a prelim (preliminary) evaluation can be done after the first 12 months of age. *CRSL provides an extra month of warranty to allow testing from the OFA and those will be accepted only if the findings of OFA is conclusive as to the condition of the dog. Failure to evaluate your dog voids any further warranty by CRSL. Remedies It is the express desire and hope that none of CRSL’s pups are afflicted with any condition under its general health or hip/elbow guarantee and/or warranty. If however, such a qualified condition of warranty occurs, we want to fully support our buyers and their pups in reaching as fair and equitable resolution as possible. Accordingly, at the sole discretion of CRSL, a pup with a disabling health condition and/or a disqualifying findings of dysplasia of the hips and/or elbows as outlined above: *At the sole discretion of CRSL, we will refund the full purchase price of the puppy upon written documentation from a licensed veterinarian setting forth the disabling health condition; and/or a written evaluation by the OFA of the presence and degree of dysplasia as outlined herein; or such partial refund as may be agreed upon; or *Provide an affected buyer with the opportunity to select a replacement puppy, of same value (or of greater value where the buyer is willing to pay the difference), from the next available litter of buyer’s choice; *In the event that the afflicted puppy was purchased from CRSL with ‘full’ AKC registration and is found to have any condition stated in Paragraph (1) above, the buyer must have the afflicted puppy spayed and/or neutered before refund of the purchase price is made - at no additional cost to CRSL; * CRSL will not accept return of any puppy found to have a disqualifying health condition and/or hip or elbow dysplasia; * All costs associated with the replacement of an afflicted puppy with respect to shipping, airfare, pre-flight health examinations, etc., will remain the responsibility of the parties as set forth in the original purchase of the afflicted puppy (same as purchasing a new puppy). Purchase, Delivery & Travel Confirmation Delivery of your puppy will acknowledge that CRSL has received full payment for the purchase of your puppy, and it has been shipped or hand-delivered to you in good health and condition. If your puppy has been shipped, you remain responsible for any outstanding costs and/or expenses that may have been incurred or advanced in your behalf, related to transportation, delivery, shipping or otherwise. If airfare, purchase price or any other cost remains outstanding after safe delivery of your puppy, you agree to make complete payment for all such outstanding amounts in not more than seven (7) days after such delivery, unless written agreement provides otherwise. If for any reason, legal or collection activity is required to obtain full restitution or payment, you agree to pay for the associated costs, including but not limited to attorney or collection fees, of all such associated efforts, and that you are not relieved of responsibility to CRSL for repayment for the entire debt, even if such amount has been submitted to a collection agency in full but said agency apportions a percentage of such recovery as its professional fee for services. Any dispute arising under the terms of this section of this Agreement of Sale shall be interpreted under the laws of the State of Missouri. AKC REGISTRATION You will be given necessary materials and information to register your pup with the American Kennel Club. You can do this online or regular mail. If you have purchased your pup with LIMITED AKC registration, you MAY NOT breed your pup upon maturity with AKC. Microchipping You can request your puppy to be micro-chipped when you take ownership. We will walk you through the steps of registering the microchip with the AKC before you leave with your puppy. Registration Name The name “Crowley's Ridge” should be contained somewhere within the puppy's full name that is filed with the AKC or any other registering organization.
